Home Visits

Patients should make every effort to consult at the surgery to make the best use of nursing and medical time. We appreciate the considerable efforts made by patients to get to the surgery for an appointment.

Home visits must be medically justifiable and not requested for social convenience.

Home visits are arranged at the discretion of the on-call clinician for patients genuinely too ill to travel to either of our two surgeries.

Where the patient, family member or carer believes a home visit is indicated, please try to telephone the surgery before 10.00. and supply the name of the patient, their address and a summary of why you are requesting a home visit.

This will assist us in ensuring the request is correctly prioritised and appropriately passed to either the on-call doctor or a member of the community nursing team or to the Duty Team at Social Services or at the Community Mental Health Service
